From my days as a level 3 support admin...

##############################

## Rest MySQL Root Password ##

##############################

/usr/bin/mysql --no-defaults --force --user=root --host=localhost --database=mysql

update user set Password=PASSWORD('tempp4ss') where User='root';

flush privileges;

stop mysql

start mysql

DON'T FORGET TO UPDATE THIS IS PHPMYADMIN (THROUGH WHM)

Hope that helps.

You might find it in one of your php applications configuration files.
